Why
This program offers your organization an opportunity to get expert assistance to move further along the integration spectrum. There is national recognition of the need for better identification of and use of evidence-based approaches to behavioral health needs in primary care settings. There is great movement across regions, states and the nation towards implementation of the various levels of integration as well as creating innovative payment structures to help support what we know has been proven to be successful treatment management models for depression, anxiety, bipolar, substance misuse and more. There is also evidence that clients with SMI do not access the medical system and have increased mortality rates. Exploring integrating or partnering with primary care clinics has proven to improve chronic disease rates for these individuals.
What Does This Mean?
The HCC, through their SIM grant, is funding direct support to primary care practices who want to implement or enhance their behavioral health integration and to behavioral health organizations who are interested in integrating primary care services to their clients. Participation in this work includes an assessment of the current level of integration in your practice, participation in a series of learning offerings tailored to your individual practice need, and access to a practice coach who can support your organization’s individual needs.
