Updates on Year 4 of the State Innovation Model (SIM) Grant

In the most recent issue of the Delaware Medical Journal, Nancy Fan, MD, chair of the Delaware Health Care Commission, reported on the progress that has been made in Year 4 of the State Innovation Model (SIM) Grant. In 2014, Delaware was a recipient of a $35 million, four-year SIM grant from the Center for Medicare & Medicaid Innovation (CMMI).

Dr. Fan provided updates on programs related to population health, including Practice Transformation (PT), Behavioral Health Integration (BHI), and the Primary Care Initiative (PCI), and detailed milestones, goal-mapping, and funding targets for Year 4 of the grant.
