Healthy Neighborhoods Statewide Consortium Meeting

Last week the Healthy Neighborhoods Statewide Consortium concluded a series of meetings that began in February and focused on four key community-health priorities: healthy lifestyles; maternal and child health; mental health and addiction; and chronic disease prevention and management. Programs and ideas from around the state were presented by Healthy Neighborhoods’ local councils and their partners.

Through these quarterly meetings, representatives from each program shared their proposed initiatives that will help support healthy communities in Delaware. The consortium comprised members from the public and private sectors, and acted as a sounding board for each local council in Delaware.

The Delaware Health Care Commission (DHCC), along with its partner, Health Management Associates (HMA), will work with local councils this summer to submit the proposed initiatives to the Center for Medicare and Medicaid Innovation, in hopes of receiving funding from the State Innovation Model grant.

We are hopeful that these initiatives will propel efforts forward to identify and build a sustainable infrastructure in each of Delaware’s counties.
